When reasonably necessary to administer an event, relevant participant information may be provided to the facilitator or service provider. We do not intend to disclose sensitive health information to facilitators unless you have provided the information for that purpose or disclosure is otherwise appropriate and permitted.
Wesleaf Mind & Body is not a healthcare provider, and our general website forms should not be used to submit confidential medical records or other sensitive medical information unless specifically requested through an appropriate process.
